Permission Forms

There are 3 forms that must be filled out and turned into the stake for each Trekker. The standard church permission slip, the application/medical release form, and the electronic devices contract.

For the standard Church permission slip please follow the Stake Youth Website link and go to "Everyone" subheading permission slips to download and print.

We would like to have these turned in at the fireside on April 12th.
